I need to make a mobile app.
1st, do you have mobile app hosting etc or do we link to your templated from within our app?
2nd, I want to be able to choose from a drop drown a person, in this case a technician, to set an appointment. When the appointment is made I want the person to be notified via sms or email.
3rd, I want to capture all this on an admin back end.

I will run down your questions in order:
1st, do you have mobile app hosting etc or do we link to your templated from within our app?
You would create your form in JotForm then embed that form to your app. Most likely using an iFrame depending on the app.

2nd, I want to be able to choose from a drop drown a person, in this case a technician, to set an appointment. When the appointment is made I want the person to be notified via sms or email.
These are our guides on setting up email notifications. The first is on setting up notifications sent to you:

The second is how to setup notifications to your users:

3rd, I want to capture all this on an admin back end.
