How do you change your name of your form and replace the photo with a new one

For your first question. On the "My Forms" page select the form you wish to change, then select "More" and click "Rename"

To update a heading picutre:

1. Open the form on edit mode.

2.  Select the heading image.

3. Click on Header Image, the wizard will show.

4. Click on the General Tab

5. Then simply remove the picture and add a new one.
