I had an enrollment application form parents were filling out for the school. I do not see it on my dashboard when I log in anymore.

May I ask for the title of the form that disappeared from your My Forms page?
I checked the Trash section but I did not find an enrollment application form there.
I also checked Favorites but none of the forms there fit your description.
The only thing close to your form is the Class Registration Form found on the top of your My Forms page.
Is this not the form you were looking for? If you have the form embedded on a webpage, please give us the URL of that page so we may check the form further. I will wait for your response.

I was unable to find a form in your account titled "App for Admissions." I also checked your Account History but I did not see any recent deletion of a form.
However, after checking the email notifications sent to your address, I found a notification that was sent from a form titled "App for Admissions." It looks like this form was created on a guest account, which is why you are unable to see it in your My Forms page. Guest accounts are created when a user creates a form before logging in to their registered account. To avoid this from happening again, please make sure that you are logged in to your Jotform account while creating the form.
For your convenience, I have now moved the form from the guest account into your OpenMindSchool account. You can now find the form in your account's My Forms page.
