I have checked your account, I can see there's a form that is currently loading empty, here's the link https://support.jotform.com/form/21493820126348
I have checked the revision history for that form and I cannot see there were recently changes, and there seems to be an issue with showing the changes dates.
I can see on the email history for that form that it had fields before.
I will forward this thread to our second level so our developers will take a look to the issue with this form and they will update us here as son as they can.
