Is there a way to be notified on the date filled in the form?

ontem mandei uma duvida e me falaram pra ver na parte de formulários medico, não achei o que eu queria.

estou precisando de um formulário que avise quando for o dia de dar remédio para o cachorro.

quero colocar no app de pet shop, para que os clientes possam se lembrados dos dias que tem que dar o remédio para o cachorro

If I understood the rest of your reply properly, you're saying you need a form that will tell the date of when to give a medicine to a dog.

Sorry but that's not something a form can do. You'd have to manually set this up on your end either by using calendar tools, notes, memos or to-do lists.

The goal of creating a form is to let your users/customers to fill it out > submit it > then you'll get the details of their submission (e.g. a date when you need to administer a medicine to their pet).
