I have had submissions completed on two of my forms this week, and neither sent me the notification email. I have also tried routing the notification email to my personal gmail address to make sure it was not the server for my business email, and the test email was not received either.
I have checked the email settings for this form http://www.jotform.us/form/60975809799177 and I can see there is missing info:
I would suggest you to set up properly your email notifications as there are some email providers that will not accept emails with missing data, so you avoid emails not being delivered because of that.

Also, when you use the "Test Email" button on the email wizard, the email is sent to the email address set up in the account, it is not sent to the email you're using as recipient.
I have checked your email address and it is not in our bounce list, an email address in the bounce list is one of the reasons why emails are not delivered.
Please if possible, ask to your email provider to white-list our IP addresses, so you avoid emails not being received because of a security filter, here is the link with our info to white-list: Whitelisting-JotMails-IP-Addresses

I found the how to prevent email bouncing related issues after I submitted the question, and did have to remove it form the bounce list. That had never happened before, but it does appear to be fixed.
Good to know you were able to resolve this.
The email address may be added to our bounce list if one email is rejected, this is why we suggest to follow that guide about preventing bouncing related issues, although there may be other reasons why emails do not get delivered.
