Does background image need to be on HTTPS to be shown on our form?

    2. Also having trouble getting background image to display. Does it have to be https image?

    If your form is opened / shared over HTTPS then you should have your background image on HTTPS server as well. The reason is that HTTPS is considered as secure if no data is loaded from it through HTTP.

    So if you add a background image that is loaded over HTTP it is very likely that the browser will decide to disable the same for you if you open your form over HTTPS.
