I checked the mail logs for your current email address and it looks like the latest submission to your "Clone of Insurance Check Up" form was sent successfully. You can also check this on your end by following this guide: How to View All Your Form Email History
However, I did notice that your previous email address rborland@*****.com was added to the bounce list, which is why you were not receiving emails there. I have removed the email address from the bounce list so you should now receive emails to that address successfully.
