Is there a way to get an image to also display in the .pdf that is generated when the form is submitted?

Last part.. we have an image in the top of the form.  When customer hits print button, it prints form like it is displayed on website.  That image is also displayed in the Autoresponder email.  Is there a way to get it to also display in the .pdf that is generated when the form is submitted?

Yes, it is possible to have the image displayed on the pdf that is generated. You can follow this guide on how to customize the PDF submission: How to Customize PDF Submissions Report.

Then from the PDF design page, simply insert your image.
