Can you please assist me in adjusting the width of my form? The text boxes and matrix? The 4th and 5th columns in my matrix are not adjusting.Also, I want the image to be still only, not moving when I scroll.

You should be able to adjust your form's width and its fields by using the Form Designer Tool. To learn more about that use of it, you can check this guide: https://www.jotform.com/help/300-Form-Designer-Tutorial-Let-s-create-fantastic-forms-
If you are familiar with CSS coding, you can also inject your own custom CSS to style your form.

Can you please check, nothing happens when I'm trying to open my Form Designer Tool.
I have checked your account and see that the Form Designer is loading fine at my end without any issues. Could you try accessing the Form Designer once again using a different browser and see if that works?
The width of the textboxes can be changed from the Form Designer as you need. Please go through the guide provided by our colleague that provides you with basic video tutorials to use the Form Designer.
You may inject the following CSS code to your JotForm to set the columns of the matrix table equal.
width: 40px !important;

Still the same issue. Also, what happened with my form? It went back to its ugly state, the one before I started editing it.
I did check your form #60326346829864 and it seems to display correctly. The width of matrix field columns are being displayed correctly and so is your form. Please check the screenshot below:
