I am not receiving email notifications from my form http://form.jotform.us/jsform/20997365127159
I have tried changing the email address the notification goes to and I have tried adding conditions to no avail. The autoresponder is working well.

It should work without condition. Could you please try to change the Sender E-mail for your notification to email@example.com? Then, check if that makes any difference.
Please also check your Spam folder.
