Why is my form is disabled

    Your account was suspended for collecting credit card information, which is strictly against our terms of use:

    In order to reactivate your JotForm account, we have deleted your form in violation. Please refrain from creating such forms in the future, and if you need to collect payments through your forms, it must be done by using one of our supported PCI DSS compliant payment processors:
