Create a form for my class schedule and registration

I need to be able to post my class schedule (MONTH BY MONTH 3 classes each month), have someone click on a class date and fill out the registration form which will show CLASS DATE NAME COMPANY PHONE EMAIL ADDRESS. The student then hits SUBMIT and it will email me. I need to be able to hit REPLY and insert a confirmation back to them.  I need to be able to edit and change class dates as needed.  Is there someone that can do this for me for a one time fee?

Unfortunately, we do not do custom or paid works for our customers. We suggest you hire someone instead or you can use one of our education form templates.

Anyway, creating a form is pretty easy. All the fields you've mentioned can be created in the form builder.

With regards to getting notifications, that's generally how the form works. You will be notified by email once someone submits your form. You can also configure your email notification to use the email field in your form as the "From Address" so once you hit the reply button in your email, your users' email address will be automatically set as the recipient address.
