I have just tested the widget on my end and it works fine:
I've checked your forms, but I could not find where you've added the widget, could you please try again?
Please also try on incognito mode, maybe this is a cache issue, check on another PC if possible.
If the issue still persists, please check the web console and share us a screenshot of that.
To open the web console follow the next keyboard shortcuts:
Console in Chrome
Mac: Command + Option + J
Windows: Control + Shift + J
Open console in Firefox
