Salesforce integration form not allowing me to map fields

    I'm on a free account, building a SFDC lead capture form. The security token shows "authenticated". However, I am unable to Match Fields for name, company, email - pulldown does not bring data. And, the Complete Integration button is faded and unclickable.

    How do I move forward?

    Yes, I reset my Salesforce security token:-)
