Account suspend?

    Hi, I just tried to log in to make an urgent change to our form and I got this message when I tried to log in:

    Due to <a target="_blank" href="https://www.jotform.com/terms" ><b><u>terms of use</u></b></a> violation, your account has been suspended.<br>It was suspected to be used for phishing. If you think this is a mistake please <a target="_blank" href="http://www.interlogy.com/contact.html?jotform=yes" ><b><u>contact us</u></b></a> to resolve this issue.

     

    Can you urgently fix this please?

    I have checked and your account humanfusion is autosuspended by our phishing system, because of the phishing form you have: http://www.jotform.co/form/61180272400846

    Inside of that form you have fields that collect Credit Card information and that is against JotForm Terms of use.

    Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

    I have removed mentioned fields from your form and I have activated your account. Please do not create forms that are against our Terms of use or your account will be suspended again and we will not be able to activate it. 

    If you want to have payment fields inside of your form you have to use one of JotForm payment tools, like you have in your other forms.

    Thank you for that. My assistant had set that form up and obviously incorrectly. We have adjusted the form correctly.   Thanks so much! :)
