How do I create a simple form for login or sign up form for my web site ?

Good morning, My name is Gary Eberhard and i'm working on a new mobile app and web site so how do I create login  or sign up form for my landing page ?

We actually do not allow for the creation of log in forms.  Since data entered in the form cannot be compare to data stored, there is no way to validate usernames or passwords.  The usernames and passwords would only be collected.  To protect users filling out forms, we do not allow for the collection of such information.  Adding such fields to a form will cause the form to be suspended.
