I have a field called "Parent's name" that autofills with the http address to the form. How did I manage to do that, and how do I make it go away?

That really is quite odd.
I would suggest deleting the field and recreating it (and variations of that) but I am worried about affecting your existing submissions.
Is it possible that for you to back up your existing submissions, so that we can try a fix like that? or is your submissions list on JotForm important to you?
Another option would be to clone the form, embed that form on googlesites, perhaps on a test page and try and fix that one.
My thought is that the field name is somehow generating the autopopulation when embedded on googlesites and my fix would involve trying to create a new field so that it won't use a similar name.
Please let us know if any of those possible routes to resolving this problem are satisfactory.

I tried deleting the field (after saving the spreadsheet to my desktop). Then I deleted the existing gadget from my site and reinstalled it. Same result--the http address autofills into the Parent's Full name field.
The address that fills in is the location of the form on Google sites, not the jotform link, so I'm not sure it's anything having to do with you.
Ok, I deleted the field again, and gave the new field a different name. For whatever reason, the problem appears to be solved.

Yeah, weird problem, must have somehow been lifting the prepopulation based on the field name . I guess there must be a query string in the googlegadget URL that has a very similar or identical name to your original field name.
Anyhow, appreciate your dilligence in working through the problem yourself.
