    I think you might try by using our Matrix Form that you can get from our Survey Tools

    You can customize the matrix to your desired results , as shown below

    In the strictest sense, no (unless a significantly edited version of the form's full source is embedded into a web page). However, I suppose a somewhat two column look can be achieved using the form builder's shrink feature which allows fields to be placed side-by-side.

    Oops! Pinoytech didn't post that I did! Earlier this evening I logged into pinoytech's account to check out how a form he posted in a thread worked. (On reflection, it would have been better if I had cloned it.) Anyway, apparently some cookies got stored on my system resulting in the above. I believe I have corrected this by following the directions in this link. (Knock wood!)

    EDIT: Wonderful! Looks like I'm me again! LOL
