Why are some fields not showing up in the e-mail to me?

    When I receive the e-mail with all the details from a filled out form, it is missing a lot of the fields even though they are REQUIRED on the form. What are the possible causes for this? Thanks for your help.

    @Tony,

    Some of the form field variables are missing on your form notification, this is the reason why some of the answers aren't presented on the email message. To fix this, simply delete your old notification and recreate a new one.

    Furthermore, just an FYI, your form is created under guest account. Guest accounts are limited up to 20 submissions only. In order to increase guest limits, I would suggest you to register an account here in JotForm using the same email address used on your notification which is info[at]h2osportsonline[dot]com.
