I am having trouble getting one of the promts to pop up on my receiving end

    I am using Jotforms through my 1 and 1 web services and I love, love, love the product but I am having trouble getting one of the promts to pop up on my receiving end.

    One of the mandatory questions the Deponent's name and that never appears when I receive the form.  I contacted 1 and 1 several times and finally they told me to contact you.  if you need to look at the page, the password is ontherecord1

    To my understanding you are referring to the content of the email you and your form respondents receive, is that correct? 

    I've searched for your form and created a clone version of it. What I found out is that the "Deponent's Name" field is not included in the contents of either your form notifier or autoresponder.

    I suggest you add another row for that field, like this:
