Now that you fixed my problem by removing my email address firstname.lastname@example.org from your server blacklist as well as making sure my email was not in your unsubscribed list (THANK YOU) ...
Can you please make sure the breeder email@example.com is not in the same situation being in the blacklist or unsubscribed list.
Kindly advise status.

This email address is in the bounce list. Here's a screenshot:
I have removed it from the list. You should be able to get new notifications now. I've checked your form notification and I can see that it is configured properly.
