    hello, my account is locked for phishing, but i don't make it.


    can you unlocked it please, i just send many email and chat, but without answers.

    I checked your form, it has a field asking for a password. You are not allowed to collect login credentials here in JotForm. Please check our Terms of Use.

    I have removed the guilty fields and re-activated your account. Please avoid asking for username, password, or credit card information. Your account should be working now.
