Form has stopped putting fields in subject of email sent to me

    I asked this question earlier, but responded to my own question and now I'm not sure it will be answered so am asking it again here.

    Hi, my form has been working correctly for a long time.  After being submitted it usually takes info from several fields and puts it in an email subject and sends it to me.  It takes the number of guests the surname and the reservation date then sends me an email with that info in the subject line.  

    ie. 2309 Jones 4

    Which equals     (date) 23rd September     (surname) Jones    (number of people) 4

    But just now it has started just putting the number of guests so the subject line of the email now reads

    4

    instead of 

    2309 Jones 4

    It is still getting the surname etc, as it appears further down the email, and in my google sheets, it has just stopped putting it in the subject line of the email it sends me. I don't think it is to do with the emails connected to my form, as it sends me a different email depending on the options selected in the form, and I have just tested and it is doing this acrooss all my emails. Hope that makes sense, any idea why this has started happening.  Thanks

     

    EDIT:  Have just checked and it has happened on all my forms.  The email sent to me when my form is submitted now just shows   {seatsRequired}    in the subject line instead of   {lastName} ddmm  {seatsRequired} 

    How can this have happenned on it's own.  I will have to go through all my emails and correct this problem manually, but

    how do I get it to display ddmm for example if the date submitted is 23.09.2016 how do I get it to input 2309 in the subject of the email sent to me.

    So I want it to be   {lastName} ddmm  {seatsRequired
