getting one form data to another

i have two simple forms you can see them both below with  the name of the field in each form and the unique ID for both i am just trying to  make it so that the data in form 1 gets passed to form 2 but am having a heck of a time gettting it to work.

can you let me know what would go after the ? when i write it ? i know the form goes ahead of the ? mark but it is the stuff after the question mark i can't get to work
