Hello,Looking to create a Human Resource onboarding form for our distributors. We would want to be able to do the following, and wanted to see if you can accommodate this via your software;Collecting basic demographic info (name, address, email, phone, etc.) Collecting Distributor relationship info (number of employees, types of physicians they call on, types of insurances those physicians see, etc) Sign forms - This is important! The ability to have them be able to fill out their w-9 and direct deposit form and sign it as part of the form. Also, the ability of them to sign their Agent Agreement. This is basically like an Adobe Echo sign feature or a Docusign feature that would be a section of the form. Ability to upload or take a photo - This would be to take a picture of a voided check for the direct deposit form.Please let me know if your software has these capabilities! Much appreciated!

Definitely, Jotform can do your requirements. The fields for collecting basic information are in the form builder. You may consider creating your form after receiving this reply.
