Recently, you responded that you were unable to migrate a form I tried to use....why can't I use this form; it is a very simple form.

I am not sure which form that you are referring to. I have checked your JotForm and see that there are 4 forms and one form is a PDF form. Please let us know which form that was unable to migrated. It would be helpful if you can provide us additional information with your previous thread or the form ID/URL so that we can guide you in the right direction.
