If a customer were to refresh this form or close the form and re-open it later, while the item quantities reset (at least visually), the calculation widget (Form Field named: "Reservation Deposit") retains old values. Same with the associated field "Due at Pickup".
What can I do to have this form truly refresh when the page is refreshed or re-loaded at a different time?

I did check your form and found that you have enabled "Auto Fill" feature to continue forms later. That is the reason why when you refresh form, the input data of form fields are still retained. Please check the screenshot below:
I would suggest you to please disable this feature and that should solve your problem.

It appears that this works with regard to fully reloading page (i.e. closing window/tab and then reopening the form). Form clears out completely.
This does not fix the problem of refreshing the form. If you toggle the quantity for one item on the form from 0 to 1, creating a deposit of $500, then refresh the window/tab, the quantity for the item returns to 0, but the deposit of $500 remains.
Please let me know if there's anything that can either:
a) retain the quantity of the item generating the deposit value or
b) return the deposit quantity to 0 when the item quantities are 0

I just did a test on your form, I put some quantities and refreshed the form, the quantities, and everything cleared:
So can you please check again and confirm for us if it is working for you?
I tested it in Chrome, did you use some other browser?

Works in Safari - refresh clears everything.
Firefox (48.0.1) on Mac retains the deposit amount while clearing the product quantities.
Not sure there's anything to be done about this - let me know.

Hi Nate, you're right, it doesn't work on Firefox 47.0.1 on Windows 10 (still worked fine on Chrome when I tested).
If I change the Inventory widget from 0 to 1 and refresh the page, the widget goes back to 0 BUT the Reservation Deposit, Due at Pickup, and Total Due Now fields retain their values.
Closing and reopening the form on the other hand reverts everything back to 0 though.
I have escalated this thread to the concerned team so they can have a closer look on why it behaves differently on Firefox. Although we can't give you any timeframe, please keep an eye out on this thread for any updates.
