Our form Daily patrol report is not being received at the designated email.
Can you give me some input on how to fix this?

I checked your form and your notification is conditionally sent, however, the condition is disabled:
If there is a condition associated with an email notification, it will not be sent unless the condition is met. Even if the only condition is disabled.
Delete this condition or setup a new notification that does not have any conditions attached.
