How can I set up a customized thank you email message based on the staff members that helped my customers?

    I have a form on my website that allows customers to refer friends to our agency. On the form, the customer has the option to select the staff member that helped them. When the customer makes a submission and chooses the staff member, I would like them to receive a Thank You email to include the staff member's contact. As if to appear that it came from the individual staff member and not the entire agency. 


    How can I do that?
