    Hi

The formatting goes out when I preview/publish the form eg mental state exam (and other sections) the fields seem to randomly move across or down?!

Also when i set the default entries and save them (eg mental health exam) some of them don't save when i publish the form...despite me re doing it several times!

    Please help!

    thanks

    Hi,

You have shrunken the fields. How many fields do you want to in a single row? If you want 3 fields, you have to move the 4th field into a new line.

    Example:

Do it on every 4th column field, that should resolve this issue.

About the default values on checkboxes, I can see them here.

    Hi

Thanks  for the responses.  With regards to the default values you can see the 'thoughts/themes'  and 'attitude' aren't checked...yet I had gone in twice and saved them all to have a default value.  Any suggestions?

With regards to the formatting ...I had wanted four fields per row and they fit when in the builder mode but not when I publish.  Something also strange happens with the other sections.

    Hello,

It seems that the reason why you're not able to view the default settings is because your form has Continue Forms Later (autofill) enabled. This causes your form to save the last information you have entered or selected

So your solution here is to either disable this feature or clear your browser's cache.

With regards to formatting, can you try extending your form's width a bit more? Often, some minor sizes in the paddings do not go exactly to the last pixel, so giving it a bit space should fix it.
