Can't find account associated with a form

I have a number of different accounts - and I cannot find where one of my forms lives. It is not in any of the accounts

That form is associated with the account username airgoddess with an email f**cruise*@gm**.com. 

You can do a password reset by clicking the "Forgot Password" in the login page.

Your email address is associated with two account, so you should probably enter the username here instead:

 

You will then receive the email instructions on how to reset your password.
