Trouble creating a new form

I duplicated one of my existing forms in order to make some changes and create a new form. I'm having trouble with the table of information that is populating from the old form to the new form. How do I insert the table that has the new questions so that we receive the correct information?

I am not sure if I have understood your question correctly. When you clone an existing form, it will copy all the form fields along with settings, conditions and css.
