Why are certain items that I have not marked with any default staying checked after being accessed in the broswer

I need immediate assistance with this as it shouldn't be happening and never has in the past.  If I cannot get this to fix I will be looking for another company to do forms with for my clients. 

This is becase you have enabled the auto-fill feature, you should disabled it by clicking on the preferences button:

Once there go to the advanced tac and set the continue forms later option to disabled:

Please do not forget to re-embed your form after doing this, otherwise the change may not be applied to your embedded version.
