How can I make the calculation of the price?

    i want to make it in a way where in the check box field only 4 is free and $3.00/extra check. so in the bottom the price is calculated with each extra they choose

    It looks like you have previously set Calculation Values to the checkbox field.

    Please remove them and replace it with the value of 1 for each options in a new line. Please see image below.

    You can then add a Form Calculation widget to your form and open the calculation wizard.

    Then setup the formula as seen below. You can follow this guide on How to Perform Calculation in the Form? (checkbox-4)*3

    Once that's done, you can now charge an additional $3 for each extra activity once they exceed 4.

    Great thanks so much

    also I want to send the form to different departments/coordinator depending whT they choose in church field. For example if they choose the first option they foes to this email and for each option is a email. That way the church they come from had a copy of the as application as well
