I used to have an "unsubscribe" button on the bottom of the of the form. What happened to it? How do I put it back on?

Is your question relevant to the subscription payment option? 

Can you please clarify what you mean with the unsubscribe button on the bottom of your form? I'm not sure what you mean exactly by that since we do not have "unsubscribe" feature unless you are only using it to capture the user information like the email address so you can unsubscribe them manually on your part.
