I tried the test through Sandbox mode which wasn't working. I also tried the Test email from the Email notifications section which sent to my email (tyler@) but I need to test a colleague receiving the notification.

If you use the "Test Email" button, the notification will always be sent to the account's email address. You need to fill up the actual form and complete the payment in order to test the notification email properly.
I can see that you are using the Authorize.net payment tool. I checked your form and I saw that the Sandbox Mode is turned off.
