Emails upon form submission are not getting forwarded to mine nor my sub-users email.
I also added a sub-user who was suppose to receive an invite via email and he hasn't received it.
How can I fix this?

Your form email is working as far as I could see it. You can check your email history to see the sent emails:
The only issue I've found here is that your Notification 1 email template is tied to a conditional logic. You have it to only send an email when Katherine is selected from the Name dropdown. Please note that the Notification 1 email won't be triggered when Katherine is not selected. Please review your settings.
You can delete the condition to send an email as it is or add a conditional logic to send an email when other options/names are selected.
