Form is Disabled

My form is disabled and I'm not sure why. Can you please check into this and let me know how to fix this.

From what I can see it looks like your account was suspended due to a suspicious form that most likely violated our terms of usage though it seems you've now deleted that particular form so I have checked your forms and lifted the suspension.
