Your account is getting suspended because some of your forms contain the word "User ID". This type words are flagged as words used for collecting login credentials. That's why your forms are rated with a high risk of being phishing forms in the system. I know you aren't asking for login credentials, but this is how the phishing system works.

I have reactivated your account I would recommend you to use another word instead of "User", or leave just the "ID" word.

Let us know if you need more help.

Please disregard my previous message, I hadn't notice that you are actually collecting login credentials:

This is something that goes against our terms of use, as explained by my colleague here: https://www.jotform.com/answers/916334

Therefor, I have suspended your account, and it shall remain suspend.

If  you delete these type of field form ALL your forms, we might consider your account eligible for re-activation.
