You may need to use the secure URL for your form. Simply replace http://www.jotform.us/ with https://secure.jotform.us to make your form secure.

For best results, we advise you to follow our guides (instead of using other untested apps) for embedding forms to your Facebook page:

ok...  thanks..... the only thing I'm confused with now is which method is meant for how I'm doing things.  I'm using the IWIPA app because it allows me to have multiple tabs and they happen ot have a free html editor too which I was using to embedd my forms from here.... so that way I can have multiple pages with multiple forms.  I have quite a few different kinds of shirts to offer and due to the way the shopping cart works I have them all as individual forms so I can create the different sizes as mutliple products.... long story short, the IWIPA app allows me to pretty much create a webpage inside of my fan page...  from what I'm getting from those links you posted those would allow me to post a single form to my fanpage... rather than all of them structured like a website...  is there anything you know would work well for that?

Did securing the form URL help in any way? If not, then the problem most likely has something to do with this IWIPA app. It seems to me that the pages you create using that app is hosted on their server - something that is beyond our control.

Try contacting their support department for further assistance.

Well, based on your previous response, it appears that that app works best for you. In that case, feel free to continue using the said application.
