I am not sure if I have understood your question correctly. Do you mean to say that you want to remove the existing form from your FB webpage and add a new one?

You have to actually remove the existing facebook tab and add again. Here are the steps to remove facebook tab:

#1. Go to facebook app page.

#2. Click on the "Settings" tab.

#3. Click on "Apps" link form left menus.

#4. Click on "x" button of JotForm app and also confirm by clicking the "Remove" button. Please check the screenshot below:
