Thank you for your help, please excuse the delayed reply, I was busy and unaware you had replied so soon.
I have managed to get the form sorted by duplicating a similar one I have done previously for slightly different purposes however it has caused uncertainty as to what information a customer has, also we don't have details we require do to complete the service. To be fair it is not the first time in the last week or so that I have experienced a glitch which is not my fault yet I have to do something to fix. Considering the numerous subsequent form views and submissions are directly due to my efforts to resolve the problem, I would feel uncomfortable with it costing me service. Would you consider implementing a means of resolve for this matter? The International Order form (not the 1.0 version) is the problem form which is the third item, still visible on my list of forms.
