The correct domain for that form is http://submit.jotform.me/form.php?formID=62014409818454&sid=348183759512141891&mode=edit 

and not http://submit.jotformpro.com/form.php?formID=62014409818454&sid=348183759512141891&mode=edit  

Unfortunately it's not possible to modify or move the form to another domain. So for this form, you should always use jotform.me.
