My forms are all disabled... how do I make them work?

    I have checked and your account with user name violet2667 was deleted today, as per your request on this thread: Also on your account violet2667 you have no forms. 

    Form that is embedded on webpage you provided is created on account with username violettatalley. That account is autosuspended by our phishing system. Your account is autosuspended because of the form "Fire & Earth Authorize Payment Form" which is located inside of Trash folder of that account. 

    Inside of your form you have fields that collect Credit Card information and that is against JotForm Terms of use.

    Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    I have purged that form from your account and I have activated your account. Form that is embedded on webpage you provided is now working.

    Also please do not crate forms that are against our terms of use or your account will be suspended again and we will not be able to activate it. 

    If you want to have payment fields inside of your form you have to use one of JotForm payment tools.
