Account Suspension

    My account has been suspended due to suspicion of phishing and I am unable to publish my form to my site. I am receiving/seeing a "Form not found" & "This form is disabled" error message. I've checked the preferences and the form is enabled. Please resolve this issue and let me know what I need to do to publish. Let me know if you have any questions.

    We do not allow for the collection of usernames and/or passwords through forms for any reason.  This is to protect users filling out forms from potential phishing.  As per our terms of use:

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    I removed the password field from your form and activated your account.  Your form should be usable once again.  If you are using your form to help set up accounts for your system, you would need to request the usernames and passwords that your users would like via email or some other medium.
