I've checked the form on Android and IOS 8 and was able to view the form. The form is also showing up on my Windows 10 PC, all browsers. Please let us know the device where the form is not showing up so we can check further.
As for the submit button, you have a condition that if the "Age Child 1" is less than 12 and greater than 14.99, the submit button will hide. You may consider removing this condition if you wish to show the button whatever the age of the Child 1:
If you need this condition, then maybe add a text field that would serve as your warning or info block telling users that age below 12 and above 14 are not allowed and cannot be submitted.

and another from an iPhone users. I made sure to inform them with the age
condition but they still can't do it. Samsung users the form don't even
open and some other users the button only doesn't show. I tried it on my
iPhone, iPad, Windows 10 Pc all worked fine. Can't find the problem as I
can't see it on my devices.

Can you please also try first re-embedding your form https://www.jotform.us/form/62037546037151 on your website using its iframe embed code instead.
User guide: Getting-the-form-iFrame-code
Make sure to remove first the previous script codes of the form on the website before adding the iframe embed code.
I have test the form itself and the condition is also working just fine.
Let us know if re-embedding the form still did not fix the issue.
