Trying to build an online order form

I need to be able to have an online order form to have Number of Members on team, Shirt style (Rip Crew/Crew) players names (First/Last) and sizes but need it to be if the select 5 players then there's 5 separate player fields, ie: player 1 info, player 2 info... and so on. 20 players selected, 20 fields. Hopefully this makes sense. Similar to the attached screenshot...

That seems to be more for if I'm selling items. I am trying to set up a template so that a "team" go order jerseys for themselves through us. I just need basics like how many team members, what sizes and their names but template needs to change based on the number of members wether it be one or 30. I'll play around some more with it. I know it's possible, just gotta find the right combination of things. Thank you!
