This is a follow-up on my email from 30 minutes ago.
On further investigation it appears that the reason why the widget no longer works is that the google maps API limit has been reached. My guess is that this is simply because so many people use this widget. This leaves you with 2 simple options:
1. Add a paid google API key to the form so it doesn't run out of credits after 2,500 daily lookups
2. Simply ask people to provide their own google maps API key when they set it up the widget
Option 1 would be a great option while you work on #2. That way we can start using the widget again instead of scrambling to find a reliable alternative.

Another support was also able to reproduce the issue and had responded to your other thread about the issue here
We have submitted a bug ticket report to the next level support already. Please refer to that thread instead for the update on the status and when fixed.
