Hello James,
Please note that you can cancel your subscription yourself: https://www.jotform.com/help/341-How-to-Downgrade-Subscription-Back-to-Free-Account
Upon your request, I have cancelled your bronze monthly subscription. You will not be billed again but your account will stay bronze until September 28, 2016 for the time you have already paid. After this date your account will automatically downgrade to free plan.
